Cascaid Clinic delivers personalized health and performance programs by combining clinical data with structured patient inputs. Prior to this project, clinicians relied on Google Forms to create and manage assessments—a solution that did not scale. Surveys were fragmented, permissions were unclear, and clinic operations teams spent significant time manually collecting, cleaning, and passing data between systems and people.

I lead the research and designs for a role-based survey builder platform to replace this workflow and centralize assessment creation and management. The system allows clinicians, coaches, and admins to create surveys based on their access level—ranging from authoring new questions to assembling assessments from a curated, shared question bank.

This platform approach introduced governance and reuse into a previously ad-hoc process. By standardizing questions and limiting unnecessary survey creation, we aimed to reduced operational overhead, improved data consistency, and avoided survey fatigue for patients.

The solution was a scalable internal tool that transformed a manual, error-prone workflow into a structured system—aligning clinical quality, operational efficiency, and user experience.
